Final Fight Guy is simply a more faithful SNES port of the original Final Fight arcade game. The first SNES port of Final Fight didn't include the third playable character, Guy, and didn't allow for co-op.
This release, which was incredibly rare in North America, includes both co-op play and all three playable characters; hence "Guy" in the title.
