From the introduction:

In the spaces between the outer planes, the Elemental Chaos, and the far flung realms beyond the reach of physical beings, entities of unusual power reign over their private corners of the cosmos. These entities, called vestiges, were once part of the natural ecosystem of the universe, but by circumstance--be it betrayal, exploration, or accident--they were ejected into the void between the planes.
